Everyone must make an online election via MyHR (company intranet or Internet), which includes enrolling you and any eligible dependents into the medical, dental and vision plan, electing your Health Care and/or Dependent Care Reimbursement annual goal amount, and confirming your 2013 ChooseWell incentive participation.
- The office visit co-pay for a specialist increases from $30 to $40 (for the EPO, PPO Low Deductible, and PPO Standard options).
- Mail order co-pays for up to a 90-day supply will increase.
The generic co-pay increases from $14 to $17.50
- The preferred brand co-pay increases from $50 to $62.50
- The non-preferred brand co-pay increases from $80 to $100.
Vision benefits will be offered as a separate, voluntary, employee-paid option. Vision coverage will no longer be automatically offered to those enrolled in the company medical plan. Employees wanting vision coverage will need to make a separate election for vision and may purchase vision benefits for their spouse even if the spouse is not covered by LG&E and KU’s medical plan.
Group legal plan
A new voluntary group legal plan will be offered through convenient payroll deductions. The cost of the plan is $20.02 monthly, and it offers participants access to a wide range of covered legal matters, most of which are 100 percent paid when you work with an ARAG network attorney.
